In accordance with state law, we are providing you with the Notice of and Consent for School Mental Health-Related Services and School Health-Related Services. These forms were sent home with your child today. They explain the medical and mental health services available to students through Maud ISD.

We kindly ask that you review, sign, and return the front page of the packet, which serves as the parental consent form.
